THE COURSE OF ENDOCRINOLOGY AT DEPARTMENT OF THERAPY TIPGPMP REPUBLICAN CLINICAL CENTRE OF ENDOCRINOLOGY
In connection with the exceptional importance of the thyroid gland problem of pathology of the organ of children is the focus of endocrinologists. Pathology of the thyroid gland in the regions of iodine deficiency, which includes the Republic of Tajikistan, occupies the leading position in the endocrine pathology in children and adolescents. Diagnostics of thyroid pathology in this category includes a phased investigation with the decision of the question concerning the goitrous increase the authority and assessment of the functional state of the thyroid gland. Many tasks successfully done with the use of ultrasonic technologies. The article suggests the use of ultrasound diagnostics for estimation of structural and functional changes of the thyroid gland, 93 children and adolescents with a duration of pathology from 6 months to 2 years. Depending on the functional state of the examined organ children and adolescents were divided into the following groups: I group amount¬ed to 61,3% of patients with diffuse nontoxic goiter, II – 20,4% of individuals autoimmune, with tireoidita III group – 18,2% of surveyed with hypopituitarism thyroid gland. Results of ultrasonic research show the greatest increase in volume of the thyroid gland in the group 2 patients with au¬toimmune thyroiditis. Analysis of results of ultrasonic research showed that in the 1 group of the examined enlargement of the thyroid gland was moderately expressed. The largest increase of the volume of the thyroid was noted in group 2 patients with autoimmune thyroiditis. Of the 17 patients with subclinical hypothyroidism last group in 58% of cases of thyroid volume slightly increased in 42% of cases, the value remains in the normal range. The results obtained allow to note that for the estimation of structural and functional changes of the thyroid gland are essential ultrasound diagnostics.

THE DEPARTMENT OF TRAUMATOLOGY AND ORTHOPEDICS TIPGPMP
The unsatisfactory results of complex treatment of skeletal injuries and injury outcomes in patients polytrauma testify to the social and economic importance. The paper analyzes the results of studying the peculiarities of the structure of multiple and associated injuries skeleton, ways to optimize the treatment of fractures on the basis of orthopedic «Damage control» of 30 trauma patients. In the structure of combined trauma of the skeleton in the 81,7% of cases, there were fractures of long bones, 36,3% – fractures of other departments, 21,3% – pelvic bones, in 5,3% of cases – fracture of the spine. Fractures of the long bones in 78,4% of cases were of indoor and 21,6% of cases – the open character. Methods of early reposition fractures in the study group used in 30,4% of the observations, delayed – 15,7%, and later – in 9,5% of cases. In the control group received digital values amounted to, respectively, of 15,4%, 11,4 per cent to 17,3 per cent. Fractures of long bones of victims of the main group in 51,8% of the observations used for conservative treatment, 48,2% – by various methods of osteosynthesis with the use of stable forms (36,5%) and methods of osteosynthesis with additional external immobilization (11,7%).

Many of the problems associated with a reduction in reproductive losses have not been ful¬filled. In the given aspect determining role belongs to the provision of contraceptive security. To understand the knowledge, attitudes and practices of women of reproductive age who use family planning services, those who are these services are provided, and also to determine the quality of contraceptive assistance to the population of medical institutions, conducted socio-logical research on the quality use of contraceptives by women of reproductive age. A specially designed questionnaire survey was conducted among 200 specialists of medical institutions for a professional assessment of knowledge, attitude and practice professionals providing contra¬ceptive services in the Republic. There was set low level of awareness of modern methods of contraception, the confidentiality of provision of contraceptive services and principles of coun-seling, post-Natal and postabortal contraception and voluntary surgical sterilization, methods of emergency contraception, indications and rules of their use, on the principles of counseling condom use. Specialists have an insufficient level of knowledge on the indication, duration of use and unwanted effects with the use of oral contraceptives and Depo-Provera.

It was spent results of echocardiography study of the heart systolo-diastolic functions at 53 patients with Ischemic Heart Disease complicated by the Complete AV-blockade. The main results of this study is that there is total IHD geneis AV-blockade depending on existence remoteness cause to essential heart function disorder in a systole and in diastole. There is tight correlative connection between the remoteness of Complete AV- blockade and left ventricle systolic function indicator, whereat diastole function passes on to heavier type.

The features of the clinical manifestations of urination disorders are their difficulty in all age periods of menopause: stress – in 54.6%, urge – in 46.6%, the imperative – to 42.9%. Anonymous survey of women at menopausal age with the use of rating scales let us to evaluate voiding disorders severity of subjective symptoms, quality of life of this category of patients and conduct performance monitoring of therapeutic measures. An integrated approach with involvement of related professionals will quickly identify reproductive tract disorders and improve this category of patients

During the period with 2006 for 2010 under supervision of authors there were 45 children with papillomatosis of laryngs in children’s ЛОР – branch NMS MZ RT by which during the first hours stay in branch in an emergency order it is spent endolaryngeal removal of papillomas of a throat. With the account of character of a clinical current papillomatosis of laryngs patients have been divided into 2 groups. In the postoperative period, patients of both groups have received viferon-2 (500 000 МЕ) on 1 суппозиторий two times a day with the 12-sentry an interval every other day. Course of treatment made 5 days. For preventive maintenance of relapses of disease 17 (70 %) the patient of 1 group and 10 (60 %) to patients of the second group the first days after operation have been entered preparations индукторы interferon (ИФН) – амиксин and циклоферон. Clinical supervision have shown that in groups where patients have received complex therapy with application амиксина and циклоферона in a combination with антиоксидантой therapy frequency of relapses of disease were reduced almost in 2 times. From here conclusions that endolaryngeal removal of throat papillomas with the subsequent use индукторов interferon and antioxidants in the postoperative period is the optimal and effective method of complex therapy papillomatosis of laryngs at children are drawn.
